Health Care seeks a compassionate and highly motivated Registered
Nurse Case Manager to join our dedicated team. As a Registered
Nurse Case Manager, you will play a crucial role in providing
exceptional care and support to our patients in the comfort of

you time!Responsibilities:- Conduct comprehensive assessments of
patients' health status and individualized needs to develop
personalized care plans.- Coordinate and manage patient care in
collaboration with physicians, therapists, and other healthcare
professionals.- Provide skilled nursing care, including medication
administration, wound care, and monitoring of vital signs.- Monitor
patients' progress and update care plans accordingly, ensuring
interventions align with their evolving needs.- Educate patients
and their families on disease management, self-care techniques, and
available community resources.- Advocate for patients' rights and
ensure their preferences and goals are integrated into the care
plan.- Maintain accurate and up-to-date documentation of patient
assessments, care provided, and outcomes achieved.- Collaborate
with interdisciplinary teams to facilitate seamless transitions of
care and improve overall patient well-being.- Adhere to regulatory
guidelines, company policies, and industry best
practices.Qualifications:- Valid Registered Nurse (RN) license in
Oregon.- Minimum two years of nursing experience, preferably in
home health or a related field.- Strong clinical assessment and
critical thinking skills.- Excellent communication and
interpersonal skills to establish rapport with patients, families,
and healthcare professionals.- Ability to prioritize and manage
multiple cases efficiently.- Demonstrated compassion, empathy, and
sensitivity in providing patient-centered care.- Knowledge of
Medicare guidelines and home health regulations is desirable.-
Proficiency in electronic health record (EHR) systems and basic
computer skills.Important:- Schedule: On call.-
License/Certification: RN (Preferred).- Willingness to travel: 75%
(Preferred).- Work Location: On the road.- Job Types: Full-time,
